Abstract: 超後期高齢者(80歳以上)肺癌の手術は周術期合併症をいかに軽減するかが術後看護の重要課題とされている.過去14年間に,80歳以上の超後期高齢者手術は19症例(2.5%)施行されていた.手術術式は年齢を考慮した縮小手術が多かった.周術期合併症は47.4%であり,内訳は呼吸機能障害が54.5%と多く,循環器障害,せん妄の順であった.1症例はMRSA肺炎にて術死し,喀痰排出介助時の清潔操作が問題であった.術後せん妄は22.2%と予想より低値であった.これらの周術期合併症を来たすことの多い超後期高齢者の看護は,高齢者家族による介護も含め,多くの問題点があり,今後の高齢者医療の課題であった.周術期を乗り切ると,十分のQOLが維持され,予後も期待出来ることより高齢者周術期看護の重要性が再認識された. / It has become a major problem to overcome perioperative complications in elderly lung cancer patients. 19 patients over 80 years old experiencing surgery were sampled. The reduction surgery was performed on 52.6% patients. There were 47.4% perioperative complications-66.7% respiratory, 33.3% cardiovascular and 22.2% postoperative delirum. One case had operative death for postoperative pneumonia because of MRSA infection due to unsterile conditions caused by a medical staff. The authors suggest that postoperative delirum has become an important subject, because of the problems in elderly care by other elderly family and proximaly to other patients. We expect associate perioperative nursing to improve prognosis and QOL.
