Abstract: 本研究の目的は,1)大学病院に勤務する女性看護師367名の心身の健康状態をPinesのバーンアウトスケールと東大式自記健康調査票THIを用いて評価すること,および2)PinesのバーンアウトスケールとTHIの12尺度・2判別値との関連を検討することによってバーンアウトの実態を理解することである.看護婦のバーンアウト平均得点は3.34でバーンアウト高得点者割合は24.4%であった.年齢の低い一般看護婦は,より高齢の者や婦長職よりバーンアウト得点が高かった.看護婦のTHI結果は精神心理的な尺度で他集団よりも得点が高く,身体的な尺度では差異はみられなかった.バーンアウト高得点の看護婦は多くのTHI尺度で高値を示した.重回帰分析では,「抑鬱性」が最も強く関連し,「年齢」が負に関連し,「多愁訴」「直情径行性」が関連していた.これらの結果より,若年層におこるバーンアウト症候群は職業に由来するDSM-3R309.00の「抑鬱気分を伴う適応障害」に近いと結論した. / This study aimed at 1) evaluating the mental and physical health status of 367 nurses working in a university hospital by using Pines's burnout scale and the Todai health index (THI), and 2) understanding the burnout status of nurses by analysing relationship of the burnout scale with the twelve scales and two discriminate scores of THI. The average burnout score was 3.34, and the percentage of burnout was 24.4%. Young nurses marked the higher burnout score than older and/or administrative nurses. Nurses marked higher scores in psychological scales of THI than other female groups, while their physical scores didn't differ with others. Nurses with high burnout scores marked higher scores in most of the THI scales. By multiple regression analysis, their burnout score correlated most strongly with the depressiveness score. It was also related with age in the negative mode, the score of many subjective symptoms, and the impulsiveness score. From these results, we concluded that burnout syndrome among young nurses was occupation-induced "adjustment disorder with depressed mood (339.00)" of DSM-III-R.
