Abstract: 膵全摘術後8年以上にわたって栄養状態を比較的詳細に観察しえた症例を経験した. 1. 糖質代謝面では,一日一回朝食前にレンテインスリンの皮下注を行なったが,血糖値の日内変動ならびに日差変動は激しく,血糖調節は困難であった.しかし,栄養状態はほぼ良好に維持された. 2. 経過中に骨粗鬆症の発生を見たが,適切な運動と栄養指導による予防対策が望まれた. 3. 慢性肝炎による肝機能障害が徐々に進行したが,脂肪肝の発生は見られなかった. 4. 術後のquality of life は概ね良好であったが,医療者側としては栄養改善の余地と退院後の社会的環境整備の必要性を感じた. / The clinical course and results of metabolic studies in a patient who had undergone total pancreatectomy eight years previously for pancreatic cancer are reported. During the first six years after pancreatectomy, the diabetic states has remained stable to a useful life at home, although there was abnormality of the high concentration of blood sugar which were measured. Her nutrition and weight are maintained on a high carbohydrate, high protein, and low fat diet. During the recent two years, she had chronic hepatitis, osteoporosis, hypoalbuminemia, anemia, and nutritional disturbance. Management of diabetes and the prevention of malnutrition and osteoporosis are the most important problems in patient care after total pancreatectomy.
