Abstract: いわゆる動く重症心身障害児(者)において歯磨きの際にみられる不適応行動について聞き取り及び行動観察による調査を行なった.その結果,歯磨きに対して強い抵抗を示すものは,全介助にて歯磨きを行なっている群の66%に認められ,その要因として,認知的に問題,また口腔内の過剰な過敏性が示唆された.また口腔内過敏と思われる一症例に対して圧迫刺激を用いた触感覚入力調整の正常化へのアプローチを試行したところ,触覚過敏の軽減に伴う歯磨きへの抵抗の減少が認められた.このように生活場面での問題行動に対して神経生理学的な理解,対処を行なうことが問題行動の軽減に際して効果的であることが示唆された, / A survey of maladaptive behavior in toothbrushing conducted with a multi-handicapped group. The procedure consisted of interviewing and behavior observation. The results showed 66% of the group who required total assistance with their toothbrushing against assistants. The reason for this resistance is thought to relate to the patient's low cognitive abilities and hypersensitivity of the oral area, as well as the surrounding facial area. In our case study, using the Sensory Integrative Approach, hard pressure was applied to the oral and facial areas of a hypersensitive patient, over a period of 5 months. This resulted in significantly reduced sensitive behavior to toothbrushing. This study shows that an understanding of the neurophysiological background of behavioral problems, and the Sensory Integrative Approach are effective in diminishing the behavioral problems related to the hypersensitivity of touch.
