Abstract: 平成14年, 厚生労働省は糖尿病患者数約740万人, その予備群を併せると約1,620万人に達すると報告した. その後平成19年国民健康・栄養調査結果ではその数が2,210万人(糖尿病患者約890万人・糖尿病予備群1,320万人)と増加の一途を辿っている. 本研究では今後A大学病院における2型糖尿病予備群の一次予防とQOL向上対策を講ずるために, 2007年4月から2008年3月の期間の同病院糖尿病非専門診療科における糖尿病予備群を抽出し, 分析及び考察した. その結果, 18歳以上でHbA_1C値が5.2%以上6.1%未満, または随時血糖値が140mg/dl以上200mg/dl未満の糖尿病予備群は2,638人であった. また, 40歳以上で糖尿病の可能性がある者(HbA_1C値が5.6%以上6.1%未満)は20.5%と厚生労働省の平成19年国民健康・栄養調査の推計16.9%を上回る結果となった. 50歳以上の年齢層では糖尿病患者数及び予備群数共に急激な増加を示した. 更に18歳から40歳代の若い年齢層における糖尿病予備群の存在も明らかとなり, 改めて糖尿病予備群の早期スクリーニングと介入及び予備群のDM発症リスク推移調査の必要性が示唆された. / In 2002, the Japanese Ministry of Health, Labour and Welfare estimated that there were 16.20 million diabetes mellitus( DM) patients in Japan, including 7.4 million diagnosed DM patients. In 2007, this number rose to 22.10 million( 8.9 million DM patients and 13.20 million suspected cases, called 'Prediabetes'). The goal of this study is to develop a program which provides primary prevention and intervention to patients at high risk for developing DM or who are in the early stages of the disease. To this end, we conducted an epidemiological study of potential DM patients who visited non-diabetes specialists at an University Hospital in Japan between April in 2007 to March in 2008. 2,638 potential DM patients were extracted from data as their HbA1C, level was above 5.2% and below 6.1%, or their blood glucose level was between 140mg/dl and 200mg/dl. Our results showed that the percentage of Prediabetes in our study is higher at 20.5% than its statistic data of 16.9% estimated in 2007 by the Japanese Ministry of Health, Labour and Welfare, in which patients were over 40 years old and their HbA1C level above 5.6% and below 6.1%). The study also found that patients who are taking steroids are at particularly high risk for developing DM. Early detection of potential DM patients and timely intervention is crucial in preventing the development of DM and protecting the QOL of these patients.
